I know from my years of counseling patients with heart disease and those at risk for heart attack and stroke that the right diet can make a huge difference in improving the chances of having a long and healthy life. 

A great diet plan is not going to do your patients any good unless they're are willing to follow them. Personal Diets consider your patients lifestyle, eating habits, and health goals. To have lasting results, patients have to make real changes in their eating habits - to develop a new way of eating instead of just "going on a diet". Long-term approach helps them make positive, effective changes to their individual diet habits with practical goals and strategies to implement.

Note about my survey: I use a scientifically validated nutrition survey to assess eating habits specifically related to fat, saturated fat, cholesterol, sodium and carbohydrate intake. This allows me to give a very objective assessment of food choices and targets the nutrients that have the most impact upon health.
